If you have evidence to believe a Board Member or staff person has committed a fraud, an illegal act, or other type of wrongdoing -- or someone is perpetrating the same against the Board -- then you should submit a report of the details to the Board.

There are several ways to submit a report using the Independent ClearView Connects service:

  1. Go to web based ClearView Connects portal and follow the instructions
  2. Call toll-free to 1-844-877-7770
  3. By regular mail to:

Reports can be made anonymously through this third-party service.

Reports are sent directly to the Vice Chair of the Board who will determine the type of investigation needed. Should a report be about the Vice Chair then it will automatically be directed to the Chair instead.
